as a Home Health RN - Deliver high-quality, patient-centered care
in the home setting - Maintain primary responsibility for assigned
caseloads, including assessment, care planning, implementation, and
evaluation - Conduct initial and ongoing nursing assessments
addressing physical, psychosocial, and environmental needs - Make
regular patient visits and re-evaluate care needs throughout the
episode - Communicate significant findings and changes in patient
condition to physicians, supervisors, and care team members -
Update and revise plans of care as patient needs evolve - Ensure
all documentation meets clinical, regulatory, and HIPAA standards -
Collaborate closely with interdisciplinary team members to support
optimal patient outcomes ? What We’re Looking For - Current,
unrestricted RN license (state of practice) - 1 year of clinical
nursing experience - Willingness to travel within the branch
coverage area (approximately 50%) - Valid driver’s license,
dependable vehicle, and current auto insurance - Associate or
Bachelor’s Degree in Nursing (preferred) - Home Health or Hospice
experience (preferred) - Experience with HomeCare HomeBase
